NHS

Our products and services do more than home monitoring. Canary Care’s expertise in healthcare and technology means NHS care workers have more tools to help them assist and assess those in care with precise and actionable data.

NHS BENEFIT

Discharge to Assess.

The Discharge to Assess (D2A) pathway requires fast, accurate and effective assessment of initial and long-term care needs, making it a perfect use case for the Canary Care system, which also facilitates ‘contactless’ assessment thereby mitigating infection transmission risks.

We have worked with numerous NHS organisations including Midlands Partnership NHS Foundation Trust.

In order to guarantee accurate assessments of patient care and self-management, we provide reliable quantitative data. This approach results in reduced DToC rates, fewer readmissions, better communication with patients and their families, and significant cost savings.

NHS BENEFIT

Accurate care. Fewer hospital trips.

The NHS are facing increasing demands and pressures following the COVID-19 pandemic. Canary Care’s innovative work with NHS organisations targets reductions in repeated unplanned health events that could lead to A&E visits and hospital admissions.

The Canary Care system can be used to identify exacerbations in long-term conditions, allowing people to receive early interventions and remain at home. This can be achieved through collaboration with NHS stakeholders such as GPs, community-based care services, and acute NHS Trusts, particularly for patients who frequently require medical attention.

NHS BENEFIT

Continuing healthcare.

Canary Care supports the objectives of NHS Continuing Healthcare (CHC) in supporting patients assessed as having a primary health need. Canary Care is used in CHC primarily in the initial assessment but also often as part of the continuing package of care.

NHS BENEFIT

Reablement

Assessing a patient's ability to take care of themselves at home has been a subjective process for a long time. Professionals can rely on their clinical experience to make these critical decisions, but with the help of Canary Care, they can now use objective data and useful insights.

Canary Care accurately tracks the daily activities of patients, which helps to avoid making decisions based on incorrect assumptions or information provided by the patients or their families. This results in patients feeling more confident and supported with care packages that are tailored to their specific needs, with reliable and accurate data for families and care givers.

NHS BENEFIT

Assessment.

One of the most difficult aspects of long-term care is assessing a patient's needs for when a care giver or family member is not around. Patients can act and behave differently when on their own, providing challenges to health services to accurately assess and provide correct safeguarding when a patient is alone.

Instead of basing these care pathway assessments on qualitative inputs or commit to labour-intensive information gathering over a number of weeks, an alternative is to deploy Canary Care to gather objective evidence that informs decisions with confidence.

Canary Care is frequently utilized by community mental health and LD teams in the NHS for assessments. It is also used occasionally for adult social care, particularly when health and social care are combined. Once data has been collected during an assessment at a service user's home, the Canary Care system can be straightforwardly uninstalled and repurposed for new assessments in other locations.

Our efforts in this field involve partnering with Surrey & Borders Partnership NHS Foundation Trust to aid in providing Older People's Mental Health Services..
